چکیده
Anne Abbott emphasises our finding that ipsilateral stroke risk is dependent on the degree of asymptomatic carotid artery stenosis.1 states this association was reported previously in ACSRS study, which she an author.2 In fact, only a non-significant correlation towards increased with stenosis found but as we showed systematic review, other much earlier studies had already more convincing correlations.3,4 However, confusion subsequently spread by absence any such associations randomised controlled trials endarterectomy for patients stenosis.

Stroke risk and critical carotid stenosis.
The risk of stroke from carotid stenosis is proportionate to the degree of stenosis, but whether this is a direct and linear relationship is unknown. Using the degree of carotid stenosis in 500 patients with asymptomatic carotid bruits as a continuous variable, we plotted the frequency distribution and related this to the risk of ischaemic cerebral events and the progression of the arterial les...
متن کاملDoes impaired cerebrovascular reactivity predict stroke risk in asymptomatic carotid stenosis? A prospective substudy of the asymptomatic carotid emboli study.
BACKGROUND AND PURPOSE Improved methods are required to identify the subgroup of patients with asymptomatic carotid stenosis who will have stroke develop. It has been suggested that impaired cerebral reactivity (CVR) may predict high risk, but no multicenter studies have examined this. METHODS In a preplanned substudy of Asymptomatic Carotid Emboli Study, 106 patients were recruited with ≥70%...

Asymptomatic Carotid Stenosis To the Editor: The recent publication of ACST confirmed the findings from a previous randomized controlled trial that carotid endarterectomy reduces the number of strokes in patients with asymptomatic carotid stenoses.1,2 These findings could be interpreted as a basis for screening for carotid artery stenoses and the widespread expansion of carotid interventions in...

A 78-year-old woman, a retired primary school principal, was referred for a second opinion regarding a recommendation that she undergo stenting of a severe carotid stenosis. A bruit in the right carotid had led to a carotid ultrasound study and a diagnosis of 80% stenosis of the right internal carotid artery. This was confirmed by computed tomogra-phy angiography, which also showed moderate ste...

Effective prevention is inarguably the best option for reducing the individual and societal burden of stroke. For each patient, clinicians balance the benefits of a given preventive therapy against its associated risks and costs. Where possible, these assessments should be based on the results of randomized clinical trials. Carotid endarterectomy (CEA), the most-commonly used surgical procedure...
